Active ingredient: Tacrolimus

Tacropic is an anti-inflammatory topical ointment containing tacrolimus that is used to treat moderate to severe atopic dermatitis.

Indications:

Tacropic is used at a dose of 0.03% and 0.1% in adults and adolescents over 16 years of age, and only 0.03% in children from 2 to 16 years of age for the treatment of atopic dermatitis (moderate and severe) in case of its resistance to other means of external therapy or the presence of contraindications to such.

Contraindications:

Hypersensitivity to tacrolimus, excipients, macrolides.

Pregnancy and breastfeeding.

Tacropic should not be used in patients with serious epidermal barrier disorders, in particular, Netherton syndrome, lamellar ichthyosis, skin manifestations of graft-versus-host disease, as well as in generalized erythroderma due to the risk of increased systemic absorption of tacrolimus.

The use of Tacropic at a dose of 0.1% is contraindicated in children and adolescents under 16 years of age, at a dose of 0.03% in children under 2 years of age.

With caution

Tacroldimus is largely metabolized in the liver, and although its blood concentration when applied externally is very low, ointment is used with caution in patients with decompensated hepatic impairment.

Caution should be exercised when using Tacropic ointment in patients with extensive skin lesions, long courses, especially in children.

How to take, course and dosage:

In adults and children over 2 years of age, Tacropic is applied in a thin layer to the affected areas of the skin. The drug can be used on any area of the body, including the face and neck, in the area of the skin folds. Do not apply to mucous membranes or occlusal dressings.

Use in children (2 years and older) and adolescents up to 16 years

Treatment should be started with 0.03% Tacropic ointment twice daily. The duration of treatment according to this scheme should not exceed three weeks. Subsequently, the frequency of use decreases to once a day, treatment continues until the lesions are completely cleared.

Use in adults and adolescents 16 years and older

Treatment should be initiated with 0.1% Tac ointment twice daily and continued until the lesions are completely cleared. As it improves, you can reduce the frequency of applying 0.1% ointment and switch to using 0.03% Tacropic ointment. If symptoms recur, reinitiate treatment with 0.1% Tacropic ointment twice daily.

If the clinical picture allows, an attempt should be made to reduce the frequency of use of the drug, or use a lower dosage - 0.03% Tacropic ointment.

Use in elderly (65 years and older)

There are no features of use in elderly people. Usually improvement is observed within one week from the start of therapy. If there are no signs of improvement during therapy within two weeks, a change in therapeutic tactics should be considered.

Treatment of relapses

Tacropic can be used for a short or long time in the form of periodically repeated courses of therapy. Treatment of the affected areas of the skin is carried out until the complete disappearance of the clinical manifestations of atopic dermatitis. Generally, improvement is seen during the first week of treatment.

If signs of improvement are not seen within two weeks of starting ointment use, other options for further treatment should be considered. Treatment should be resumed when the first signs of exacerbation of atopic dermatitis appear.

Exacerbation prevention

To prevent relapses and increase the duration of remission in patients with a history of frequent (more than 4 times a year) relapses, maintenance therapy with Tacropic is recommended.

The appropriateness of maintenance therapy is determined by the effectiveness of previous treatment according to the standard regimen (2 times a day) for no more than 6 weeks.

For maintenance therapy, Tacropic ointment should be applied 2 times a week (eg, Monday and Thursday) to areas of skin commonly affected by exacerbations.

The time interval between application of the drug should be at least 2-3 days.

In adults and adolescents 16 years and older, 0.1% Tacropic ointment is used, in children (2 years and older) - 0.03% Tacropic® ointment. If signs of relapse occur, switch to the usual Tacropic ointment regimen (see Relapse Management).

After 12 months of maintenance therapy, it is necessary to assess the clinical dynamics and decide whether it is advisable to continue prophylactic use of Tacropic. In pediatric patients, treatment should be temporarily discontinued and continued maintenance should be considered to assess clinical dynamics.
